Qualifications

• Must have a valid Minnesota Driver’s License with an acceptable record.
• Ability to pass DHS background study.

Responsibilities

• Provide client-centered case management services for CADI/BI & DD waivers.
• Meet with clients 2-3 times per year and manage caseloads of 35 to 43 clients.
• Serve as an advocate and connect clients to community resources.

Education

Bachelor's Degree in social work, special education, psychology, nursing, or related field
