Fraser in Minnesota is seeking Case Managers to support CADI/BI and DD waiver clients. You will advocate for individuals, link them to community resources, and maintain thorough case notes.

Caseloads include mostly adults with some children, with 2–3 client meetings per year and occasional travel within Ramsey County. Requirements: Bachelor's Degree in social work or related field; Minnesota driver’s license; ability to pass DHS background check; and commitment to diversity and inclusion.

Competenze

• Bachelor's Degree in social work, special education, psychology, nursing, human services or related field.
• If degree is in social work, social work licensure is required.
• Minnesota Driver’s License with acceptable record.
• Ability to pass DHS background study.
• Commitment to promoting diversity, multiculturalism, and inclusion.

Mansioni

• Provide client-centered case management for CADI/BI waiver and DD services.
• Maintain caseloads (about 35–43 clients) and meet with clients 2–3 times per year.
• Advocate for Fraser clients and connect them to community resources.
