Qualifications

• 2-4 years in healthcare field or medical/health setting
• Experience in call center or customer service environment
• Able to work independently and meet daily metrics with speed, accuracy and a caring presence telephonically
• Demonstrates ability to meet daily metrics with speed, accuracy, a positive attitude, and strong written and oral communication skills
• Able to navigate multiple systems including Microsoft Office 365, clinical platforms, and health plan systems
• Technically proficient with computer systems and digital tools
• Ability to collaborate with clinical teams
• Excellent communication with providers and internal stakeholders
• Commitment to delivering high-quality service and patient care outcomes

Responsibilities

• Coordinate and triage Care Team tasks and care plans
• Screen patients and identify needed medical services
• Refer to medical services and coordinate required services under the benefit plan
• Promote effective utilization of healthcare services
• Support development and implementation of care plans under direction of nurse or medical director
