Job summary

CVS Health is seeking a Care Management Associate to support coordination of medical services, triage, and care plan implementation. The role emphasizes efficient use of healthcare resources, collaboration with nurses and clinical staff, and maintaining HIPAA compliance.

Training Schedule anytime between 8:00 am - 7:00 pm EST. You will use Med Compass and other systems to enter member information, refer to specialty programs, and promote effective care management.

Qualifications

• 2-4 years of experience in healthcare, medical billing or coding.
• Experience in call center or customer service environment.
• Ability to work independently while meeting deadlines with a caring presence telephonically.
• Strong written and oral communication skills.
• Proficiency navigating multiple systems including Microsoft Office 365 and clinical platforms.
• Ability to collaborate with clinical teams.

Responsibilities

• Coordinate and triage Care Team tasks and care plans.
• Support implementation of care plans to promote appropriate utilization of services.
• Identify and refer to appropriate medical services, monitor cases and coordinate services.
• Communicate with healthcare providers and internal team members to ensure effective care.
• Document case information and support non-medical research as needed.
