Job summary

CVS Health is seeking a Care Management Associate to coordinate medical services, support care plans, and triage Care Team tasks. You will identify needed services, refer to appropriate resources, and document care in EHR systems under nurse or medical director guidance.

You will collaborate with providers and care team members to promote efficient, high-quality patient care while working from a designated home office within your state.

Qualifications

• 2-4 years of experience healthcare field, medical/health setting, medical billing, and coding.
• Experience in call center or customer service environment.
• Works independently and competently, meeting deliverables and deadlines while demonstrating an outgoing, enthusiastic and caring presence telephonically.
• Demonstrates ability to meet daily metrics with speed, accuracy, a positive attitude, and strong written and oral communication skills.
• This role requires the ability to work efficiently across multiple systems simultaneously, including Microsoft Office 365 applications, clinical platforms, and health plan systems.
• Technically proficient, with the ability to navigate computer systems and digital tools efficiently.

Responsibilities

• Support comprehensive coordination of medical services and care plans.
• Triage Care Team tasks and refer to appropriate services.
• Coordinate care under nurse or medical director guidance and document details.
• Utilize Med Compass and other systems to enter and retrieve member information.
• Promote communication with providers and care team members to improve outcomes.
