Job summary

CVS Health is seeking a Care Management Associate to support comprehensive coordination of medical services for members. You will triage care team tasks, identify needs, and coordinate referrals, documenting decisions in the care management system.

In this role, you’ll collaborate with nurses and physicians, promote effective care plans, and ensure timely access to services while maintaining HIPAA standards. Training is provided with schedules between 8:00 am–7:00 pm EST.

Qualifications

• 2-4 years in healthcare, medical setting or coding/billing
• Experience in call center or customer service environment
• Ability to work independently and meet daily metrics
• Strong written and oral communication skills
• Experience navigating multiple hospital/healthplan systems

Responsibilities

• Triage Care Team tasks and identify admission/referral needs
• Coordinate and communicate care plans with providers and care team
• Support implementation of care plans and appropriate service utilization
• Document reviews and coordinate services while maintaining HIPAA compliance
• Identify opportunities to improve patient access to services and outcomes
