Job summary

Humana Inc. is seeking a remote Care Management Assistant to support care management programs by coordinating administrative, operational, and patient-facing activities.

This role reports to the Associate Director, Consumer Experience and requires independent judgment in handling care needs for members. You will collaborate with members, providers, and care teams to review records, prepare care summaries, and inform next steps to improve patient outcomes.

Qualifications

• High School Diploma or GED.
• Experience providing in-center or virtual patient support.
• Experience in patient support, care coordination, or healthcare setting.
• Experience with data entry and maintaining information in an EMR system.
• Can work EST or CST.

Responsibilities

• Coordinate assessment, coordination, and facilitation of members' care needs to support health outcomes.
• Collaborate with members, providers, caregivers, and care teams to support member wellbeing through care coordination and care summaries.
• Apply discretion and judgment in prioritizing requests and interpreting procedures.
• Utilize prior care management experience to perform the role.
