Job summary

Humana / CenterWell is seeking a Care Management Assistant to coordinate administrative, operational and patient support activities for care management programs. You will assist in assessment, coordination, and facilitation of members' care needs to support health outcomes.

You will collaborate with members, providers and care teams, review patient records, and prepare care summaries to guide next steps and improve patient wellbeing. Remote role with travel as needed.

Qualifications

• High School Diploma or GED required.
• Experience providing in-center or virtual patient support.
• Experience in patient support, care coordination, or healthcare setting.
• Experience with data entry in an EMR system.
• Ability to work EST or CST.

Responsibilities

• Coordinate administrative, operational and customer support activities supporting care management programs.
• Support assessment, coordination and facilitation of members' care needs to improve health outcomes.
• Collaborate with members, providers, caregivers and care teams to inform care decisions and prepare care summaries.
• Use discretion to prioritize requests and apply established procedures within care management processes.
