Humana Inc. invites you to join its CenterWell Senior Primary Care team as a Care Management Assistant, coordinating administrative, operational, and patient support to improve health outcomes.

You will collaborate with members, providers, and care teams, manage care plans, and support care coordination with EMR data entry, risk assessment, and patient record maintenance. Remote work with occasional travel for training.

Qualifications

• High School Diploma or GED.
• Experience providing in-center or virtual support or interaction with patients.
• Experience in patient support, care coordination, or a related healthcare setting.
• Experience with data entry and maintaining information within an electronic medical record (EMR) system.
• Can work EST or CST.

Responsabilités

• Coordinate administrative, operational, and customer support activities for care management programs.
• Collaborate with members, providers, caregivers, and care teams to support member wellbeing and inform care decisions.
• Use discretion in prioritizing requests and applying established procedures.
• Leverage experience in care management processes to perform responsibilities.
