Job summary

Humana, Inc. is seeking a Care Management Assistant to coordinate administrative, operational, and patient support activities that back care management programs and services.

You will perform advanced tasks with independent initiative and report to the Associate Director, Consumer Experience. You will support care coordination, review patient records, prepare care summaries, and collaborate with members, providers, and care teams to improve outcomes.

Qualifications

• High School Diploma or GED.
• Experience providing in-center or virtual support or interaction with patients.
• Experience in patient support, care coordination, or a related healthcare setting.
• Experience with data entry and maintaining information within an electronic medical record (EMR) system.
• Can work EST or CST.

Responsibilities

• Coordinate administrative, operational, and customer support activities for care management programs.
• Support assessment, coordination, and facilitation of members' care needs to achieve health outcomes.
• Collaborate with members, providers, caregivers, and care teams to inform care plans and improve outcomes.
• Apply discretion and judgment prioritizing requests and following established procedures.
