Job summary

Humana Inc. is seeking a Care Management Assistant to coordinate administrative and operational support for care management programs. You will work with members, providers, and care teams to enhance well-being and outcomes, using your care management experience to prioritize and execute tasks with discretion.

The role is remote with occasional travel to offices for training or meetings. Expected weekly hours are 40, with a pay range of $43,000–$56,200 per year.

Qualifications

• High School Diploma or GED required.
• Experience providing in-center or virtual patient support.
• Experience in patient support, care coordination, or related healthcare setting.
• Experience with data entry and EMR systems.
• Ability to work in EST or CST time zones.

Responsibilities

• Coordinate administrative, operational, and customer support activities for care management programs.
• Collaborate with members, providers, caregivers, and care teams to support wellbeing and improved outcomes.
• Apply discretion and judgment in prioritizing requests and following procedures.
• Leverage prior care management experience to perform duties effectively.
