The role reports to the Associate Director, Consumer Experience and involves coordinating administrative, operational, and patient support tasks to help patients achieve health outcomes. You will collaborate with members, providers, and care teams to review records, coordinate care, and prepare care summaries that guide actions and improve outcomes.

Qualifikationen

• High School Diploma or GED.
• Experience providing in-center or virtual support or interaction with patients.
• Experience in patient support, care coordination, or a related healthcare setting.
• Experience with data entry and maintaining information within an electronic medical record (EMR) system.
• Can work EST or CST.

• Coordinate administrative, operational, and customer support activities for care management programs and healthcare services.
• Support the assessment, coordination, and facilitation of members' care needs to help achieve and maintain health outcomes.
• Collaborate with members, providers, caregivers, and care teams to support member wellbeing through care coordination and care summaries.
• Use discretion and judgment in prioritizing requests, interpreting and adapting procedures, and applying established techniques.
• Apply prior experience of care management processes and healthcare operations to perform the role.
