Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (nurse or physical therapist) to review and approve patient information at start of care, recertification, resumption of care, or evaluation visits. You will partner with branch leadership to ensure documentation meets regulatory standards and to train LP staff.

This role requires a valid clinical license, CPR, a year of experience in Medicare home health or hospice, and a valid driver license.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license and CPR certification.
• Minimum of one year of experience in Medicare home health or hospice.
• Valid driver's license and automobile liability insurance.
• Proficient with Microsoft Word and Excel.

Responsibilities

• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Provide ongoing education and training to branch LPs on documentation and regulatory standards.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers, and staff.
