Job summary

Enhabit Home Health & Hospice seeks a licensed professional to review and approve patient information across care transitions, ensuring compliance with documentation standards. The role partners with branch leadership to educate staff and uphold quality patient care while maintaining strong relationships with referral sources, patients and caregivers.

The candidate should hold an approved nursing or physical therapy degree with an active license, CPR certification, and reliable transportation.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Active nursing or physical therapy license; CPR certification.
• Minimum one year of experience; Medicare home health/hospice preferred.
• Valid state driver's license and automobile liability insurance.
• Strong technology skills and ability to use mobile devices.

Responsibilities

• Review and approve patient information during start of care, recertification, resumption of care or evaluation visit.
• Partner with branch leadership to train LPs on documentation and regulatory standards.
• Provide in-depth training to LPs as needed; promote quality patient care and maintain relationships with referrals, patients, caregivers and staff.
