Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to review and approve patient information at start of care, recertification, resumption of care, or evaluation visits. The role partners with branch leadership to deliver training and ensure documentation meets regulatory standards.

Requires graduation from an approved school of nursing or physical therapy, active license, CPR certification, valid state driver's license, and reliable transportation.

• Graduate of an approved school of professional nursing or physical therapy.
• Active state license to practice.
• CPR certification.

Responsibilities

• Review and approve patient information for start of care, recertification, resumption of care, or evaluation visit.
• Train LPs and provide ongoing education to ensure documentation meets regulatory standards.
• Promote quality patient care and maintain relationships with referrals, patients, and staff.
