Job summary

Enhabit Home Health & Hospice is seeking qualified licensed professionals to review and approve patient information at start of care, re-certification, or evaluation visits. You will collaborate with branch leadership to educate LPs and ensure documentation compliance, while promoting high-quality patient care and strong relationships with referrals and families.

Required are a nursing or physical therapy degree, an active license, CPR, a valid driver’s license, and basic Microsoft Office

Qualifications

• Must be a graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements to practice.
• Minimum one year of experience.
• Medicare home health or hospice experience preferred.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Knowledge of Microsoft Office (Word/Excel).

Responsibilities

• Review and approve patient information during start of care, re-certification, resumption of care, or evaluation visits.
• Provide ongoing education and training to branch LPs on documentation requirements.
• Meet with LPs for in-depth training when needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers, and staff.
