Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (nurse or physical therapist) to review patient information and approve care plans during initial and ongoing visits. You will train LPs, ensure documentation meets regulatory standards, and promote high-quality patient care across branches.

Applicants should have an active state license, CPR certification, and at least one year of relevant experience, preferably in Medicare home health or hospice settings.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must maintain a valid clinical license as required by state regulations.
• Minimum of one year of experience.
• Prior Medicare home health or hospice experience preferred.
• CPR certification required.
• Valid state driver license and automobile liability insurance required.
• Basic technology skills including use of a mobile device.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Review and approve patient information submitted by the LP during start of care, recertification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs to ensure documentation meets regulatory standards.
• Conduct one-on-one training when needed and promote quality patient care.
• Maintain strong relationships with referral sources, patients, caregivers and staff.
