Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to review patient information at start of care, recertification, resumption of care, or evaluation visits. You will collaborate with branch leadership to train LPs on documentation and regulatory standards, ensuring quality patient care.

The role emphasizes ongoing education, strong communication with referrals, patients, caregivers, and staff, and maintaining compliance in a dynamic home health setting.

Qualifications

• Licensed nursing or physical therapy degree with active license.
• Minimum one year of experience in Medicare home health or hospice setting.
• CPR certification and a valid state driver’s license with automobile liability insurance.
• Strong technology skills and familiarity with mobile devices.
• Experience with Homecare Homebase (HCHB) is preferred.

Responsibilities

• Review and approve patient information submitted by LP during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation and regulatory requirements.
