Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (RN or PT) to review and approve patient information across start of care, recertification, resumption of care, or evaluation visits. You will partner with branch leadership to educate LPs and ensure documentation meets regulatory standards.

When needed, you will conduct one-on-one trainings to uphold quality patient care. In-office collaboration is essential to Enhabit objectives; CPR, licenses, and a valid driver’s license are

Qualifications

• Graduate from an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements.
• Minimum of one year of experience.
• CPR certification required.
• Valid state driver license and automobile liability insurance.

Responsibilities

• Review and approve patient information for start of care, re-certification, resumption of care or evaluation visits.
• Partner with branch leadership to educate LPs on documentation and regulatory standards.
• When needed, conduct one-on-one training with LPs.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
