Enhabit Home Health & Hospice is seeking an experienced licensed professional to review and approve patient information during care start, recertification, and evaluations. The role involves partnering with branch leadership to train LPs on documentation and regulatory standards, including one-on-one training when needed.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license as per state requirements.
• Minimum of one year of experience in a Medicare home health or hospice setting.
• CPR certification required.
• Valid state driver license and automobile liability insurance.

Responsibilities

• Review and approve patient information submitted by LP during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation and regulatory standards.
• Meet one-on-one with LPs for in-depth training when needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
