Job summary

Enhabit Home Health & Hospice invites qualified nurses or physical therapists to join our team in a role focused on reviewing and approving patient information at starts of care, re-certifications, resumption of care, and evaluations.

You will partner with branch leadership to educate LPs, ensure documentation meets standards, and provide targeted one-on-one training when needed. A Medicare home health or hospice background and CPR certification are preferred.

Qualifications

• Required graduate of an approved school of professional nursing or physical therapy.
• Required to have and maintain a valid clinical license, per state requirements to complete this job in accordance with applicable board's rules.
• Experience as a licensed professional may be deferred with a review and approval from the vice president operations or regional president.
• Required to have minimum of one year of experience.
• Required previous experience in a Medicare home health or hospice setting.
• Required to have and maintain CPR certification, as required by position.
• Required to have and maintain a valid state driver license in the current state of residence
• Required to have and maintain automobile liability insurance, as required by law
• Preferred previous experience with packet review / quality improvement.
• Preferred previous experience with coding.
• Required to have basic demonstrated technology skills, including operation of a mobile device.
• Preferred previous experience using Homecare Homebase (HCHB).
• Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
• Oral communication, written communication, fluency in English, active listening.
• Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.

Responsibilities

• Responsible for the review and approval of all patient information submitted by the Licensed Professional (LP) during a start of care, re-certification, resumption of care, or evaluation visit.
• Partners with branch leadership to provide ongoing education and training to all branch LPs to ensure staff understand documentation requirements in an effort to meet regulatory standards and to keep staff abreast of changes.
• When necessary, meet one-on-one with individual LPs to provide in-depth training.
• Promotes quality patient care; ensures stable relationships with referral sources, patients, caregivers and employees.
