Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to review patient information, train LP staff on documentation, and ensure regulatory standards are met. The role emphasizes ongoing education and strong collaboration with branch leadership to maintain quality patient care.

Ideal candidates will have a nursing or physical therapy background, valid state licensure, CPR, and Medicare home health or hospice experience, with proficient technology skills and reliable transportation.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Current valid professional license in state of residence.
• Minimum one year of experience in Medicare home health or hospice setting.
• CPR certification is required.
• Valid state driver license and automobile liability insurance are required.
• Experience with packet review/quality improvement and coding is preferred.
• Proficient basic technology skills including Microsoft Word and Excel.
• In-office attendance is essential (except for designated full-time remote positions).

Responsibilities

• Review and approve patient information submitted by LP during start of care, re-certification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs on documentation requirements to meet regulatory standards.
• Conduct one-on-one training when necessary and promote quality patient care while maintaining relationships with referral sources, patients, caregivers and staff.
