Job summary

Enhabit Home Health & Hospice is seeking an experienced professional to review and approve patient information submitted by Licensed Professionals during starts of care, recertifications, and evaluations. You will train LPs to meet regulatory standards and promote high-quality care.

Ideal candidates have a nursing or physical therapy background, active licensure, CPR, a valid driver license, and Medicare home health or hospice experience.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements to practice.
• Minimum of one year of experience.
• Experience in a Medicare home health or hospice setting.
• CPR certification and active status.
• Valid state driver license and automobile liability insurance.
• Basic technology skills including use of a mobile device.

Responsibilities

• Review and approve patient information submitted by the LP during start of care, re-certification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate LPs on documentation requirements to meet regulatory standards.
• Meet one-on-one with LPs to provide in-depth training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
