Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to review patient information and support LPs during care initiation and evaluations. You will collaborate with branch leadership to ensure documentation accuracy and regulatory compliance while promoting high-quality patient care.

The role emphasizes training, adherence to standards, and maintaining strong relationships with patients, families, and referral sources.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license as per state requirements.
• Minimum one year of experience in a Medicare home health or hospice setting.
• CPR certification.
• Valid state driver's license and auto insurance.
• Basic technology skills including MS Word and Excel.

Responsibilities

• Review and approve patient information submitted by LP during start of care, recertification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs on documentation requirements and regulatory standards.
• One-on-one training sessions when needed to improve performance and accuracy.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
