Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to review and approve patient information submitted by Licensed Professionals during start of care, recertification, resumption of care, or evaluation visits.

You will partner with branch leadership to educate LPs on documentation requirements, provide in-depth one-on-one training when needed, and promote high-quality patient care while maintaining strong relationships with referrals, patients, caregivers and colleagues.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must maintain a valid clinical license per state requirements.
• Experience in Medicare home health or hospice preferred.
• CPR certification and valid state driver's license required.
• Strong technology skills and use of mobile devices.

Responsibilities

• Review and approve patient information submitted by LPs during care events.
• Partner with branch leadership to educate and train LPs on documentation requirements.
• Conduct one-on-one training with LPs when needed to ensure compliance.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
