Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to review and approve patient information during care starts, recertifications and evaluations. You will train LPs on documentation, ensure regulatory compliance, and promote high-quality patient care while maintaining strong relationships with referral sources and families.

The role requires an approved nursing or physical therapy education, an active state license, CPR certification, and a valid driver's license with insurance.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must hold a valid clinical license per state rules.
• Minimum of one year of experience in healthcare.
• Previous Medicare home health or hospice experience preferred.
• CPR certification required.
• Valid state driver license and automobile liability insurance required.
• Basic technology skills: Microsoft Office and mobile device use.
• Homecare Homebase (HCHB) experience preferred.
• Strong English communication and active listening skills.

Responsibilities

• Review and approve all patient information submitted by the Licensed Professional during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation requirements to meet regulatory standards.
• Provide one-on-one training with LPs when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
