Enhabit Home Health & Hospice is seeking a licensed professional to review patient information submitted by LPs during start of care, certifications, and evaluations. You will partner with branch leadership to provide ongoing education and training on documentation requirements to meet regulatory standards.

Qualifikationen

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license in state of practice.
• Minimum of one year of experience in home health or hospice setting.
• CPR certification.
• Valid driver license and automobile liability insurance.
• Basic technology skills including mobile device use.
• Experience with Homecare Homebase (HCHB) preferred.

Aufgaben

• Review and approve patient information submitted by the Licensed Professional (LP) during start of care, re-certification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs to ensure documentation compliance with regulatory standards.
• Conduct one-on-one training with LPs when needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
