Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (RN or PT) to review and approve patient information at start of care and during recertification, ensuring regulatory compliance.

You will train LPs, collaborate with branch leadership, and promote high-quality patient care while maintaining strong relationships with referral sources, patients, and staff. In-office attendance is essential.

Qualifications

• Required graduate of an approved school of professional nursing or physical therapy.
• Must maintain CPR certification and a valid state driver license.
• Excellent English communication and interpersonal skills.

Responsibilities

• Review and approve patient information at start of care, re-certification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate LPs and ensure documentation meets regulatory standards.
• Provide one-on-one in-depth training for LPs as needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
