Job summary

Enhabit Home Health & Hospice is seeking a skilled Licensed Professional to review and authorize patient information at start of care, recertifications, and evaluations. You will partner with branch leadership to educate LPs on documentation and regulatory changes, providing in-depth training as needed.

The role emphasizes promoting high-quality patient care, maintaining relationships with referral sources, patients, caregivers and employees, and ensuring staff compliance with evolving standards.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must maintain a valid clinical license per state requirements.
• Minimum of one year of experience in Medicare home health or hospice.
• CPR certification and valid state driver license required.

Responsibilities

• Review and approve patient information submitted by the LP during start of care, recertification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs to ensure regulatory compliance and up-to-date documentation.
• Conduct one-on-one training when necessary and promote quality patient care with diverse stakeholders.
