Job summary

Enhabit Home Health & Hospice is seeking licensed professionals (RN/PT) to review patient data for care starts, recertifications, and evaluations. You will work with branch leadership to train LPs on documentation and regulatory updates, occasionally meeting one-on-one for in-depth coaching.

A focus on high-quality patient care and strong relationships with referral sources, patients, and caregivers is essential.

Qualifications

• Must graduate from an approved school of nursing or physical therapy.
• Must hold a valid clinical license per state regulations.
• Minimum of one year of experience; Medicare home health or hospice preferred.
• CPR certification required; valid driver license and automobile liability insurance.
• Proven ability to work with documentation and regulatory standards.

Responsibilities

• Review and approve patient information for start of care, re-certification, resumption of care, or evaluation visits.
• Collaborate with branch leadership to educate and train LPs on documentation requirements.
• Provide one-on-one training when needed and ensure staff understand regulatory changes.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers, and employees.
