Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (nurse or PT) with Medicare home health or hospice experience to review patient information and ensure regulatory compliance. You will train LPs on documentation and support branch leadership while maintaining strong relationships with referral sources and families.

The role requires CPR certification, a valid driver's license, and reliable transportation to operate in all weather conditions. Office presence is essential.

Qualifications

• Must be a graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements to practice.
• Minimum one year of experience in Medicare home health or hospice.
• CPR certification maintained.
• Valid state driver license and automobile liability insurance.

Responsibilities

• Review and approve patient information submitted by LP during start of care, re-certification, resumption of care or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation to meet regulatory standards.
• Provide one-on-one training when needed.
• Promote quality patient care and maintain relationships with referrals, patients, and caregivers.
