Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (nursing or physical therapy) to review patient information during start of care and other visits, and to train LP staff on documentation requirements.

You will partner with branch leadership to ensure regulatory-compliant practices, promote quality patient care, and maintain relationships with referrals, patients, caregivers and employees. A valid license, CPR, and driver’s license are required.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license and CPR certification.
• Minimum of one year of experience.
• Experience in Medicare home health or hospice setting.
• Valid state driver's license and automobile liability insurance.
• Strong technology skills, including Microsoft Office.
• Oral and written communication with English fluency.

Responsibilities

• Review and approve patient information submitted by LP during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to provide ongoing education and training to all branch LPs.
• Meet one-on-one with LPs to provide in-depth training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
