Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to review and approve patient information at start of care, re-certification, or evaluation visits. You will collaborate with branch leadership to educate LPs and ensure documentation meets regulatory standards.

This role emphasizes quality patient care, maintaining relationships with referral sources, patients, caregivers and staff. In-office attendance is essential to support supervision and teamwork across branches.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements.
• Minimum of one year of experience.
• Experience in Medicare home health or hospice setting preferred.
• CPR certification and a valid state driver license; automobile liability insurance.
• Basic technology skills including Microsoft Office.
• In-office attendance is essential for non-remote roles.
• Preferred experience with packet review/quality improvement and coding.

Responsibilities

• Review and approve all patient information submitted by LP during start of care, re-certification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate LPs and ensure documentation meets regulatory standards.
• Provide one-on-one training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers, and employees.
