Job summary

Enhabit Home Health & Hospice seeks an experienced Licensed Professional to review patient information during start of care, recertifications and evaluations. You will train LPs, ensure documentation meets standards, and uphold strong relationships with referrals, patients and caregivers.

A current nursing or physical therapy license, CPR, and a valid driver’s license are required. This role emphasizes in-office collaboration, education, and adherence to Medicare guidelines while supporting

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid state clinical license required to practice.
• Minimum one year of experience in healthcare.
• Medicare home health or hospice experience preferred.
• CPR certification required and maintained.
• Valid driver's license and auto liability insurance required.
• Basic tech skills with mobile device proficiency.
• Experience with packet reviews/quality improvement preferred.
• Experience with coding is a plus.

Responsibilities

• Review and approve patient information submitted by LP during start of care, re-certification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs to ensure documentation meets regulatory standards.
• Meet one-on-one with LPs when needed for in-depth training.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
