OneWell Health Care is seeking a Per Diem Employment Specialist in Abington, PA. You will support individuals with IDD to achieve integrated employment, focusing on career planning, job placement, coaching, and long‑term retention in community settings.

Responsibilities include assessments, individualized plans, job search assistance, and collaboration with employers. Must have a high school diploma; a BA in human services is preferred. Per Diem schedule offers flexible hours and local travel.

Qualifications

• High school diploma or equivalent is required.
• Bachelor’s degree in human services or related field preferred.
• Experience with individuals with IDD is preferred.
• ACRE or CES P certification within 6 months of employment is required.

Responsabilités

• Develop individualized employment plans and support job placement.
• Provide on‑the‑job coaching, resume and interview prep, and workplace readiness training.
• Identify employment opportunities and build employer partnerships.
• Monitor progress, ensure regulatory compliance, and document services.
