Job summary

OneWell Health Care in Reading, PA seeks a Per Diem Employment Specialist to support individuals with IDD in achieving competitive employment. This field-based role emphasizes career planning, job placement, coaching, and long-term employment retention through person-centered services.

You will travel to participant homes and community sites, perform assessments, build employer partnerships, provide on-the-job coaching, and maintain compliant documentation.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree in human services or related field preferred.
• Experience working with individuals with IDD preferred.
• CESP or ACRE certification required within 6 months of employment.

Responsibilities

• Conduct vocational assessments and develop individualized employment plans.
• Support individuals in securing and maintaining competitive employment (job search, resume, interview prep, workplace readiness).
• Build and maintain employer partnerships and provide on-the-job coaching and onboarding support.
• Monitor progress, address barriers to employment and retention, and ensure documentation meets regulatory requirements.
• Advocate for accommodations and promote disability awareness and inclusion.
