Job summary

OneWell Health Care is seeking a Per Diem Employment Specialist to support individuals with IDD in achieving competitive, integrated employment within the community across Pennsylvania. The role emphasizes career planning, job placement, coaching, and long-term retention through person-centered services.

Responsibilities include assessments, plan development, employer partnerships, on-the-job coaching, and ensuring accurate documentation and compliance with ADA/ODP and other regulations.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree in human services or related field preferred.
• ACRE or CESPrequired within 6 months of employment.
• Experience with IDD populations is preferred.

Responsibilities

• Conduct vocational and career assessments and develop employment plans.
• Support individuals in securing and maintaining competitive employment.
• Provide on-the-job coaching, resume/interview prep and workplace readiness training.
• Maintain documentation, reports, and regulatory compliance.
