OneWell Health Care is seeking a Per Diem Employment Specialist to support individuals with IDD in achieving competitive employment across Pennsylvania. The role focuses on career planning, job placement, coaching, and long‑term retention through person-centered services.

The position operates in community settings with flexible scheduling and requires direct interaction with participants, employers, and interdisciplinary teams. A valid driver’s license and local travel are expected.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree in human services or related field preferred.
• Experience with IDD/IDD services preferred.
• Background in supported employment or vocational services preferred.

Responsibilities

• Conduct vocational and career assessments and develop individualized employment plans.
• Support individuals in securing and maintaining competitive employment through job search assistance, resume and interview preparation, and workplace readiness training.
• Build and maintain employer partnerships; provide on-the-job coaching and onboarding support; monitor performance.
• Advocate for workplace accommodations, promote disability awareness and inclusion; document progress and ensure regulatory compliance.
