OneWell Health Care is seeking a Per Diem Employment Specialist in Elizabethtown, PA to help individuals with IDD achieve competitive, integrated employment in the community. You will conduct assessments, create plans, and provide job search support, coaching, and retention strategies in field settings.

The role emphasizes flexible scheduling, working with employers and interdisciplinary teams, and ensuring compliance with ADA and state regulations while promoting inclusion and independence.

Qualifikationen

• High school diploma or equivalent required.
• Bachelor’s degree in human services or related field preferred.
• Experience with IDD or vocational services preferred.
• CESP or ACRE certification required within 6 months of employment.

Aufgaben

• Conduct vocational assessments and develop individualized employment plans.
• Support individuals in securing and maintaining competitive employment.
• Provide on-the-job coaching, resume and interview preparation, and workplace readiness training.
• Maintain documentation, ensure regulatory compliance, and monitor progress toward goals.
• Build employer partnerships and coordinate with benefits counselors and interdisciplinary teams.
