Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays. You will be a key liaison among patients, families, and the hospice team, delivering compassionate care and coordinating the plan of care.

The role requires an active RN license, 1+ year nursing experience, and comfort working in patients’ homes or care facilities.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• A hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and the ability to provide support in emotionally intense settings.
• Comfortable providing care in patients’ homes, facilities, or wherever they reside.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsabilités

• Provide direct patient care and support after normal business hours.
• Serve as the primary liaison between patients, families, physicians, and the hospice team during the shift.
• Respond promptly to urgent calls, patient visits, and symptom management needs.
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
• Maintain accurate and updated medical records and interdisciplinary notes.
• Participate in quality assessment and performance improvement programs.
