Job summary

Heartland Hospice is seeking an After Hours/On-Call Registered Nurse to provide compassionate care in patients' homes during evenings, nights, weekends, and holidays. You will coordinate with the daytime team and act as the primary liaison with families, physicians, and hospice staff during your shift.

This safety-sensitive role requires an active RN license, CPR, and at least 1 year of nursing experience, preferably in hospice or home health.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year of nursing experience.
• Hospice, home health, or palliative care exposure preferred.
• CPR certification.

Responsibilities

• Respond to patient needs after hours with direct patient care.
• Serve as primary liaison between patients, families, physicians, and hospice team during shifts.
• Provide urgent call response, patient visits, and symptom management.
• Document clinical visits and communications accurately.
• Coordinate with interdisciplinary team to ensure continuity of care.
