Heartland Hospice is seeking an After Hours RN to respond to patient needs during evenings, nights, weekends, and holidays, providing compassionate care when it is most needed. You will work closely with an interdisciplinary team, document visits, and help guide clinical decisions as the primary liaison for patients and families.

Ideal candidates hold an active RN license, have 1+ year nursing experience, CPR certification, and a hospice-centered approach.

Qualifications

• Current RN license in the state of employment or eligibility to obtain.
• 1+ years of nursing experience.
• CPR certification is required.
• Hospice mindset with compassion and calm under pressure.

Responsibilities

• Provide direct patient care and support after normal business hours.
• Serve as primary liaison between patients, families, physicians, and the hospice team during shifts.
• Document clinical visits and communications accurately in EMR.
• Participate in quality assessment and performance improvement programs.
• Maintain continuity of care and coordination with daytime care team.
