Job summary

Heartland Hospice is seeking an After Hours/On-Call RN to provide compassionate care in patients' homes and care facilities after evenings, nights, weekends, and holidays. You will respond to urgent needs, coordinate with the hospice team, and guide the plan of care with strong clinical judgment.

This role emphasizes independent work and seamless handoffs to daytime staff. Requirements include an active RN license in the state, 1+ year nursing experience, CPR, and reliable transportation.

Qualifications

• Current RN license in the state of employment (or eligibility to obtain)
• 1+ years of nursing experience
• Hospice heart: compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and ability to provide support in emotionally intense settings
• Comfortable providing care in patients’ homes, facilities, or wherever they reside
• Valid driver’s license, automobile insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Respond to patient needs after normal business hours with direct patient care
• Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift
• Respond promptly to urgent calls, patient visits, and symptom management needs
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care
• Document clinical visits and communications accurately
• Participate in quality assessment and performance improvement programs
• Work independently while coordinating with the daytime care team
