Job summary

Heartland Hospice is seeking an After Hours/On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays. You will provide direct care, coordinate with the hospice team, and support families in the home setting.

Ideal candidates hold an active RN license, have 1+ years of nursing experience, and demonstrate strong clinical judgment and compassionate communication. A valid driver’s license and CPR certification are required.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year of nursing experience.
• Hospice mindset: compassionate, calm under pressure.
• Comfort providing care in patients' homes or facilities.
• Valid driver’s license, auto insurance, reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays after hours.
• Provide direct patient care and support after normal business hours.
• Serve as liaison between patients, families, physicians, and hospice team during shift.
• Document visits and communications accurately in EMR.
• Assess patient needs and guide care plans with the Interdisciplinary Group.
• Maintain continuity of care and coordinate with daytime team.
