Job summary

Heartland Hospice is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, providing compassionate care in patients' homes and care settings. You will report to leadership while coordinating with the daytime team to ensure seamless, high-quality care.

The role emphasizes independent practice with strong clinical judgment, comprehensive patient assessments, and education for patients and families, all within the Hospice Plan of Care and EMR

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year of nursing experience.
• CPR certification required.
• Comfort delivering care in patients' homes or similar settings.
• Strong clinical judgment and communication.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to patient needs after hours, evenings, weekends, and holidays.
• Provide direct patient care and support during shifts.
• Serve as liaison between patients, families, physicians, and hospice team.
• Document visits and communications accurately in EMR.
• Evaluate patient and family needs to guide care plans.
• Collaborate with Interdisciplinary Group to deliver holistic care.
• Participate in quality assessment and improvement programs.
