Job summary

Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care in patients' homes. You will coordinate with the hospice team, document clinical visits, and support symptom management while ensuring continuity of care.

Ideal candidates have an active RN license, 1+ year nursing experience, CPR certification, and strong clinical judgment.

Qualifications

• 1+ years of nursing experience

Responsibilities

• Respond to patient needs after hours and weekends
• Serve as liaison between patients, families, physicians and hospice team
• Document visits and communications
• Provide direct patient care and symptom management
