Job summary

Heartland Hospice is seeking an After Hours / On-Call Hospice Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care and ensuring continuity of care.

You will report to the Administrator or Patient Care Manager Senior, provide direct patient care after hours, and coordinate with the interdisciplinary team to guide the plan of care and support families.

Qualifications

• Current RN license in the state of employment is required.
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and ability to provide support in emotionally intense settings.
• Comfortable providing care in patients’ homes or other residence settings.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays.
• Provide direct patient care and support after normal business hours.
• Serve as the primary liaison between patients, families, physicians, and the hospice team during shifts.
• Document clinical visits and communications accurately.
• Participate in quality assessment and performance improvement programs.
