Job summary

Heartland Hospice is seeking an After Hours / On-Call RN to respond to patient needs during evenings, nights, weekends, and holidays. You will act as the primary liaison between patients, families, physicians, and the hospice team, delivering compassionate, high-quality care in home settings.

You will document visits, manage symptoms, and participate in quality improvement while collaborating with interdisciplinary staff. A valid RN license and CPR certification are required.

Qualifications

• Current RN license in the state of employment (or eligibility to obtain).
• 1+ year of nursing experience.
• Hospice heart: compassionate, calm, and dependable in urgent care situations.
• Valid driver’s license and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays.
• Serve as primary liaison between patients, families, physicians and hospice team.
• Provide direct patient care and symptom management after normal business hours.
• Document clinical visits and communications accurately.
• Participate in quality assessment and performance improvement programs.
• Coordinate with daytime care team to ensure continuity of care.
• Maintain infection control measures and standard precautions.
