Job summary

Heartland Hospice is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays in home-based settings. You will act as the primary liaison and ensure continuity of care with the daytime team.

The role requires an active RN license, 1+ year of nursing experience, and CPR certification. A compassionate hospice approach and strong clinical judgment are essential for success in high-demand, emotionally charged environments.

Qualifications

Current RN license for the state of employment
1+ years of nursing experience
Hospice heart: compassionate, calm under pressure
Excellent clinical judgment and ability to support in emotionally intense settings
Comfortable delivering care in patients' homes or residence settings
Valid driver’s license, auto insurance, reliable transportation
Current CPR certification

Responsibilities

Report to Administrator, Executive Director, or Patient Care Manager Senior
Provide direct patient care and support after normal business hours
Serve as primary liaison between patients, families, physicians, and hospice team
Respond promptly to urgent calls, visits, and symptom management
Evaluate patient and family needs to guide clinical decisions
Collaborate with LPNs/LVNs, CNAs, and support staff
Document clinical visits and communications accurately
Participate in quality assessment and performance improvement programs
Work independently while coordinating with daytime team
