Job summary

Focused Post Acute Care Partners in Texas seeks an MDS Coordinator to assist the Director of Nursing by ensuring that documentation meets federal, state, and certification guidelines. The coordinator will manage the MDS, CAAs, and interdisciplinary care plans to support accurate payment.

Responsibilities include tracking the RAI process, coordinating care conferences, ensuring signatures, and compliance with OBRA, PPS, and Medicare/Medicaid requirements.

Qualifications

• Must hold an active LVN license and nursing experience in skilled nursing facilities.
• Strong knowledge of the RAI process, PPS, and Medicaid reimbursement.
• Familiarity with OBRA regulations and the care plan process.

Responsibilities

• Coordinate the RAI process from MDS to care plan completion.
• Maintain ongoing documentation to meet federal/state guidelines.
• Ensure timely submission of MDS data to state and federal agencies.
